CT Forms and Instructions for Legal Name/Gender Marker Changes

Step 1: Probate Court – Change of Name

*Both the affidavit and petition must be filled out/submitted; waiver is optional and must meet criteria (see form)

Step 2: Social Security Administration – Change of Name/Gender Marker

·      Adult Name Change on Social Security Card: https://www.ssa.gov/pubs/EN-05-10513.pdf

*Can also request to change gender marker upon issuance of new card without supporting documentation; proof of name change through probate court order required

 Step 3: Driver’s License/Registration – Change of Name/Gender Marker

Registration:

*Name change must be filed with Social Security Administration BEFORE changing name on driver’s license (SSA may take up to 48 hours to process the change)

Step 4: Birth Certificate – Change of Name/Gender Marker

*For CT birth certificates, you will need a letter from a physician, APRN, or psychologist to change your gender marker on your birth certificate; for just a name change, you will only need your probate court order

 Step 5: Passport

*In both cases, you must submit certified documentation of legal name change (i.e., probate court order)
